Vital Records

Apache County Office of Vital Records issues certified copies of birth and death certificates occurring In the state of Arizona. For all other vital records services, please visit the Arizona Department of Health Services Bureau of Vital Records website.

Records Request

To request a birth or death certificate, please complete the appropriate online form and print a copy to mail or bring to us in person. We also have forms available at the counter. 

We accept records requests in person or via mail. Please plan to pay the $20 fee per record with a debit or credit card or money order payable to “Apache County Vital Records.” We do not accept cash or personal checks. You may also order records online through the Arizona Department of Public Health Services.

Native American Delayed Birth Certificate Registration

If you are a Native American born before 1970 and have an unregistered birth, you will need to apply for a delayed birth certificate. Please visit the Arizona Department of Health Services website and follow the instructions provided to register a delayed birth certificate

Our office cannot create or print delayed birth certificates (certificates issued more than one year after a birth).

Monday through Thursday (closed Fridays) 

We are closed one week each month for community outreach services. Please call ahead before arriving—especially if you are traveling a long distance.
